Every Sport Club Athlete is required to submit a pre-participation clearance form signed by a physician each year. You can do this in two ways:
- Have your own outside physician sign the Participation Clearance Form for each year you participate.
- Schedule an appointment through Student Health Services to receive clearance. The benefit of having Student Health Services conduct your physical is that you are only required to have the one initial physical, then each year thereafter you can submit an online questionnaire instead to be reviewed by a physician. Pending the review, you will the receive clearance without having to schedule a visit in person.
NEW ATHLETES
For new Sport Club Athletes, or Sport Club Athletes with their own outside physician:

If you choose to complete your clearance through Student Health Services, please call 805-893-3371 to schedule an appointment. Print, fill out, and bring the following forms with you to your appointment. The fee (subject to change)for this physical exam is currently $75.00 and will be charged to your BARC account.

RETURNING ATHLETES
For Sport Club Athletes who have already completed an initial physical with Student Health Services:
Your pre-participation health clearance can be completed by submitting an online health history form to Student Health. You must complete this requirement by the end of fall quarter 2011.
This clearance process can take up to one week, and you will be directed to pick up your clearance at Student Health when it is ready. A $30.00 fee will be charged to your BARC account for this clearance.

- Click on the blue box in the upper left corner of the home page
- Log-in using your UCSB Net ID and password
- Enter the first six digits of your perm
- Click on “Messages” on the left column
- Click “New Message”
- Select Sport Club Returning Athlete Clearance and click on continue.
- Complete and submit your Clearance form
